Yes, the delivery of health care services is different in Russia, but don't let this discourage you. I'm familiar with many nurses whom received their education training in the foreign country, but they were able to pass the American exams and receive the nursing credentials. I'm guessing the greatest challenge to passing these exams, is proficiency in English and familiarization with our medical terminology.

With that being said, I think every man should consider relocating to the country of the foreign spouse at some point. Whether as a personal option, or if the unforseen crisis should occur on the national scale. But as others have pointed out, it makes the most sense that a Russian woman could adapt most easily to the foreign country. For example, my medical profession is not recognized outside of North America. In order to receive employment in the Russian hospital, it would be necessary to pursue additional medical education and training in Russia, instead of passing an exam.
